I have read and understand the contract documents for this plan, which include the Terms of Service, Electricity Facts Label and Your Rights as a Customer documents. I have been given the opportunity to print and save copies of these documents which are provided above. I understand that after submitting this plan change request, I will also receive a copy of these documents, including the Terms of Service, from Discount Power via email, and upon request, via US mail, and that these documents, including the Terms of Service, explain all the terms of the agreement. I understand that if I terminate my service before my contract ends on my new plan, a cancellation fee may apply. I understand that if I do not take action to ensure the continued receipt of electricity service at the end of this plan’s initial term (for example by enrolling on a new plan), I will continue to be served on a month-to-month basis as described in my plan’s Terms of Service.
